How do I log into the Fountainhead PPP application?
To log into your Fountainhead application either click the link sent to you via email or navigate to the site here. The site will prompt you to enter the email address used to set up your account. After you enter your email, you will be sent a 4-digit verification code directly to your email. You will need to enter this 4-digit verification code into the system before you move forward for security purposes. Once entered, you will be logged directly into the dashboard for your account.

Once on the dashboard for your account, check the status in the column marked ‘status’ – if it says ‘submitted to verifier’ it has been submitted. If it does not have a status, this means that the application has not been submitted and additional information/steps are necessary to move forward.
How do you search your email to find messages from Fountainhead?
To search for email messages from Fountainhead, simply navigate to the search function inside your email and type in Fountainhead (one word). This will pull up any and all correspondence from your lender.

What is the email address that Fountainhead will use to send correspondence about my application?
All emails associated with Fountainhead about PPP will come from the address info@fountainheadcc.com Here’s a couple of tips per email carrier on how to make sure these do not end up in your spam.
Gmail
Desktop – in the upper right-hand corner click on the 9 dots next to your profile icon, in the drop-down select contacts, click ‘create contact’ in the upper left corner and then ‘create a contact’. Enter in the first name as Fountainhead and the email above, click save
Mobile – inside the mobile application, click on the 3 lines in the upper left corner. Select ‘contacts’ from the drop-down under the section Google Apps. Click the blue plus sign in the lower right-hand corner and enter in the first name as Fountainhead and email address as shown above).
Yahoo
Click the contacts icon on the right-hand side of the inbox, scroll to the bottom of the drop-down and select ‘new contact’, enter in their first name as Fountainheadand email address as shown above.
Hotmail
In the Windows Live Hotmail main window, click the Contacts link, this will take you to the Contacts page. Select ‘new’ and enter in the first name as Fountainhead and the email as shown above.
Outlook
Select the ‘people’ icon in the lower left-hand corner, then ‘new contact’ and then enter in the first name as Fountainhead and the email as shown above.
What documents will I need to provide for the Fountainhead PPP application?
- Entity Formation Documents, if applicable
- Driver’s License (or passport) on all owners of 20% or more of the borrowing entity
- 2019 or 2020 IRS Form 940/1120S (whichever year the borrower chooses to base their loan on)
- Payroll summary report (or W2’s on all employees) for the corresponding year, which reflect all employee annual wages, taxes withheld, etc. (in order to comply with SBA’s guidelines to filter-out compensation above $100k for loan amount calculations)
- Any other corresponding documentation that you wish to include in the loan amount calculation, such as receipts for employees’ health insurance paid by the company
